The A&S Outprocessing form should be filled out by the departments for all exiting faculty, staff, and instructors. The purpose of these records is to make sure that the individuals return all technology and pertinent items, and that the departments have contact information (address, phone, and email) in case we should need to get in touch with them after they are gone. The document can be completed manually or electronically. All completed forms should be signed by the department chairs and stored in a secure location within the department.
